When a client creates an account, we only ask for basic information: name, email, phone, address, and age.
The health questionnaires come later, when a client requests a prescription. This is standard practice for telehealth, as a clinician needs a clear picture of someone's health before prescribing. At that point the client completes questionnaires specific to the prescription they're requesting, along with our general questionnaires (annual and monthly) that help establish a baseline. These may ask about current health status, past medical history, medications, allergies, family history, and other relevant information.
Together, these give the clinician the detail they need about what the client is requesting, so they can review it and prescribe when appropriate. They're required, and an order can't move forward without them, an incomplete questionnaire may result in the order being cancelled.
Check-in questionnaires come after a client has started a medication. They're a simple way for the clinician to see how the client is doing, whether they're noticing side effects, seeing the results they're after, or have questions about their dosage. We encourage clients to complete these so their clinician can stay current on how the treatment is going.
