Why do we call it a "service fee" and not a "commission" or "margin"?

Important distinctions.

Because that's what it is. You're being compensated for the services you provide to your clients — not for referring them to a clinical provider.

This is not an affiliate program. Affiliate programs pay you a percentage of someone else's revenue in exchange for sending a lead. That is not what happens here.

Here's what actually happens on Altro:

  • Your client receives clinical services from an independent clinical organization and pharmacies (prescribing, dispensing, clinical oversight). Those services are billed and paid for separately.

  • You provide your own services to your client — care navigation, coaching, education, nutrition support, exercise guidance, accountability, and any other wellness support you offer as part of your practice.

  • The service fee is your compensation for your services. It reflects the value of the ongoing support you deliver to your clients.

Because it's compensation for your services, you set your own service fee. You can price it at any level that reflects the value of your work — including at zero if you choose. That decision is yours because these are your services to price.

This structure allows wellness professionals to build sustainable, high-touch practices while their clients receive clinical care through appropriately licensed providers.
